Performance Levels 1-12, Roosevelt : Saturday, March 14, 2009.

Directions for the ISMTA AIM Examination Enrollment Form:

1. Circle Theory Levels 3-12 OR Performance.

2. Circle your Audition Center.  Use two forms if you plan on enrolling students at more than one location.

3. Write in the Date of Examination.

4.  Fill in your Name, email, and address.

5. Enter the starting time and ending time of your judging availability.

6. Enter your preferred judging levels.

7. Enter your students Last Name, First Name, School (if applicable), City of School, Level, and Fee.

8. Total the fee and send the check with the form to the event chair.

Directions for the Performance Rating Sheet:

1. Copy/download this form as needed (one per Levels 1-3 students and two for all other levels)

2. Write in student's name and level. (Ignore "Teacher Code, Studio, Date, and Time)

3. Circle Test location.

4. Write in the Title and Composer for your student's repertoire.  Include all relevant information such as opus number, K., BWV, etc. 

5. Send completed sheets to the Performance Chair by March 2, 2009.
